The Sylvan-Westgate building is just off US-26 and west of the Oregon Zoo. My office is located on the first floor, on the East side of the building; it is the second office on the left from the East facing door. When you arrive for your appointment, please come in to the waiting room and I will be out to get you at the start of our appointment.
